AlohaCare Community Conscience Awards to Fund Hawaii Community Health Improvement Programs
AlohaCare, a nonprofit health plan which specializes in caring for those who are medically underserved and medically fragile, is accepting applications for the AlohaCare Community Conscience Award, a grant program specifically for Hawaii nonprofit organizations working to improve the health of Hawaii's communities.
Each year AlohaCare awards a total of $30,000 in grants to Hawaii nonprofits. Applicants must be a nonprofit 501(c)(3) organization or have an equivalent tax-exempt IRS status. (This award excludes community health centers, which AlohaCare is currently supporting through its quality improvement initiatives.)
Project must focus on health and wellness — educate others on the prevention and/or treatment of asthma, child and adolescent well-care visits and immunizations, diabetes, obesity, and/or women's health; access to care — reduce barriers to health care, such as lack of transportation, lack of health insurance, language or cultural barriers, and shortage of providers in rural areas; and/or aged, blind, and disabled populations — benefit the overall health and wellness of Hawaii's kupuna or those who are medically fragile and medically disadvantaged.
Awards are for a one-year period.
