Blue Cross and Blue Shield of Minnesota Foundation Seeks Proposals for Building Health Equity Together Grants

The Blue Cross and Blue Shield of Minnesota Foundation is seeking grant proposals for Building Health Equity Together, a new program to support partnerships of local governments and nonprofit organizations working to achieve health equity in their community.

Project partners should include two different departments in a local unit of government (county, statutory or home rule charter city, township or school district) working with a 501(c)(3) community-based organization to advance a common agenda related to influences on health in low-income communities. The partnership should address one or more of the factors that influence health outcomes including education, employment, income, family and social support, and community safety. Tribal governments in partnership with a 501(c)(3) community-based organization are eligible.

The foundation will award two grants of up to $75,000 with the possibility of a second year of funding at the same level. First year funds may be used for planning and/or implementation of a concrete initiative that partners implement together. Eligible costs include project staff salaries/benefits, activity costs, project-related equipment/supplies, and other project-related direct costs. Administrative costs for the grant budget should not exceed 10 percent. Project activities may occur anywhere in the local unit of government or tribal government community. Second year funding, conditional upon first year performance, may be used for implementation of plans completed in year one.

Proposals will be accepted online beginning August 27, 2012, and are due by September 28, 2012.
