Chicago Community Trust Seeks Grant Proposals for Programs to Improve Access to Health Care
The Chicago Community Trust has released a Request for Proposals for projects that will increase or maintain access to affordable healthcare services for residents of metropolitan Chicago.
The trust will support efforts led by the public sector and/or nonprofit agencies to improve access to health care for low-income families and individuals. In advance of the full implementation of the Affordable Care Act, the trust seeks results in the Chicago region that will provide 1) a well-integrated, highly functioning public health system that incorporates federal, state, county, and city resources; quality standards and technology to improve medical service delivery; strategies to reduce duplication of services; and services for primary and specialty care; and 2) improved access to care for the uninsured.
To be eligible, applicants must be nonprofit 501(c)(3) organizations or those using a 501(c)(3) fiscal agent; be located within and/or primarily serving residents of Cook County, except for regional, statewide, or national projects or research that may benefit a substantial portion of Cook County residents; have a commitment to diversity and inclusion for their governance, staffing, and populations served; explicitly adhere to non-discriminatory practices in the hiring of staff or in providing services on the basis of race, religion, gender, sexual orientation, age, national origin, or disability; and demonstrate a successful track record in addressing the needs of low-income residents.
The trust will make between five and ten grants through this RFP. Grants will be in the range of $35,000 to $150,000. Successful applicants will receive grants for a minimum of one year.
