Novo Nordisk Invites Grant Applications for Programs to Help People Living With Type 2 Diabetes in Detroit
Novo Nordisk is accepting applications for its Community Care Detroit initiative, which will provide $80,000 in funding for programs designed to educate people in the metro Detroit area living with type 2 diabetes.
The Novo Nordisk Community Care initiative provides support for organizations that are ambassadors for change in diabetes care and are making an impact in U.S. communities most affected by type 2 diabetes.
In 2011, the community care program sponsored more than twenty diverse community-based programs and awarded $415,000 to organizations in Atlanta, Minneapolis, Boston, and Philadelphia. Last year's sponsored programs were selected for their ability to provide unique content focused on patient education and other resources, including interactive workshops, material for underserved and minority populations, and faith-based community initiatives.
The 2012 program is offering new sponsorship opportunities to eligible nonprofit organizations dedicated to educating and empowering patients in Detroit to become more engaged in their health and management of type 2 diabetes.
Only organizations with nonprofit 501(c)(3) status are eligible. (Government bodies, hospitals, clinics, pharmacies, and other provider organizations are not eligible.) The proposed program must focus on adults already diagnosed with type 2 diabetes in the metro area of Detroit (Lapeer, Livingston, Macomb, Oakland, St. Clair, and Wayne counties). Programs focused on health care providers, juveniles, youth, type 1 diabetes, or pre-diabetes are not eligible.
Requests cannot exceed $20,000. Novo Nordisk must be the exclusive and sole provider of funds for the program. Programs eligible for sponsorship must start in 2012 but may extend into 2013.
