Robert Wood Johnson Foundation Announces Video Contest Winners

The Robert Wood Johnson Foundation in Princeton, New Jersey, has announced the five winners of its Transitions to Better Care video contest.

Launched as part of the RWJF-sponsored Care About Your Care initiative, a month-long effort designed to focus attention on the national problem of hospital readmissions, the contest invited practitioners, patients, and family members to submit videos showing an innovation that helped patients successfully transition out of the hospital. Studies have found that about one in five elderly patients released from a hospital is back in hospital within thirty days — and more than a third are back within ninety days — and that return visits could be avoided with better care transitions upon a patient's discharge.

Selected from a pool of more than a hundred applicants, the winners are the Cullman Regional Medical Center in Alabama; Mercy Health of Cincinnati; Northern Piedmont Community Care in Durham, North Carolina; Queen of the Valley Medical Center in Napa, California; and the University of Utah Health Care in Salt Lake City. The winners will join the foundation at a Care About Your Care event later this month, where they will receive special recognition and a professionally produced video describing their innovation.

"These videos provide examples of how hospitals, doctors, nurses, and other providers across the country are developing innovative ways to improve care for patients," said RWJF senior program officer and team director Anne F. Weiss. "The contest winners and all those who participated should be proud of the important work they are doing to effectively coordinate care with their patients and other providers."
