Horizon Foundation Awards $2 Million to Nonprofit Health Centers in New Jersey
The Newark-based Horizon Foundation for New Jersey has announced grants totaling $2 million to thirteen federally qualified health centers, free/volunteer clinics, and independent clinics serving uninsured populations in the state through the second phase of its Health Center Initiative.
On the heels of a five-year, $5 million commitment by the foundation to reduce costly emergency room visits that could affect some 340,000 patients, phase-two grants ranging from $50,000 to $250,000 were awarded to help community health centers make facility enhancements, service improvements, and increase the number of uninsured patients receiving care. Projects to be funded include the creation of a school-based health center, improving access to dental services for the uninsured, converting paper records to an electronic system, and transforming a facility into a patient-centered medical home. In addition, several centers will create approximately two dozen new full- and part-time jobs with the funding.
"Horizon has a long history of supporting charitable and community organizations that make a positive difference in the lives of [New Jersey residents] all across our state," said Horizon Foundation chair Robert A. Marino, who also serves as chair and CEO of Horizon Blue Cross Blue Shield of New Jersey. "The primary and preventative services delivered through these community centers will keep patients healthy and dramatically reduce their use of hospital emergency rooms for non-emergency care. The foundation grants will strategically support centers that expand their services and focus on the patient-centered medical home model of care."
