Chicago Family Health Center - Patient Centered Medical Home Manager
Chicago Family Health Center (CFHC), founded in 1977, is a Federally Qualified Health Center providing quality primary care, oral health, behavioral health and social support programs to medically underserved and uninsured residents living in far south and southeast parts of Chicago in Cook County, Illinois. With a total population of 379,195, the CFHC primary service area comprises eleven Chicago Community Areas including South Chicago, Roseland, Pullman, South Deering, East Side, West Pullman, Riverdale, Auburn Gresham, Chicago Lawn, West Lawn, and West Englewood. Thirty-five years ago, CFHC’s staff of five served 400 patients in a small South Chicago storefront. In 2011, CFHC provided comprehensive primary medical, dental, behavioral health care to 28,000 patients through 114,000 visits. CFHC health services are accessible and utilized primarily by African American and Hispanic populations, indicative of CFHC staff experience with the cultural and linguistic characteristics of our patients and the communities where they live. Cultural sensitivity, competency, and diversity training are essential for CFHC staff and inform our efforts to provide accessible healthcare to every patient.
POSITION SUMMARY
The Patient Centered Medical Home Manager will serve as a facilitator and catalyst for improving the clinical and operational performance using the Patient Centered Medical Home (PCMH) model of care. He/she will be a subject matter expert on NCQA and JCA requirements, Meaningful Use, and Open Access Scheduling. He/she will be knowledgeable in systems improvement and have the ability to design and implement multifaceted projects and facilitate a “Patient Centered Medical Home” (PCMH) culture.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Develop and implement the National Council on Quality Assurance's (NCQA) Patient Centered Medical Home (PCMH) recognition program.

Attain expert knowledge of NCQA PCMH recognition standards, policies, processes, and application requirements.

Develop and implement best practices to improve workflow related to medical home standards

Develop and implement a training program to align workflow and organizational culture with NCQA standards

Oversee and develop policies and procedures necessary for PCMH implementation

Research, gather, and report monthly health related data to meet requirements of NCQA model and funding agencies

Oversee the collection, integration and evaluation of diverse data related to clinical and operational performance and medical home transformation activities

Participate, facilitate, and lead other inititives that align with the PCMH model including The Joint Commmission (TJC) and Meaningful Use

Lead the CFHC PCMH Leadership Group and related committees including the development of meeting agendas and the preparation of minutes in a timely manner

Provide information and recommendations needed for decision making and resource planning

Develop and implement care management and care coordination standards

Work collaboratively with Clinical, Operations, and Information Technology Departments to implement PCMH elements

Provide regular program updates, program needs, and outcomes as requested by CFHC leadership
REQUIREMENTS
Applicant must have 2-5 years experience working in a clinical setting with demonstrated knowledge of ambulatory clinic operations and community health. A Master's Degree in Nursing, Public Health, Health Administration, or Business Administration is preferred. Additionally, the ideal applicant will have strong interpersonal skills, familiarity with quality improvement programs and health information technology including Meaningful Use. Proven administrative and management ability in complex environments, including supervisory experience. Demonstrated performance in program development and implementation. Demonstrated ability to analyze and utilize data. Excellent oral and written communication skills as well as innovative ideas, flexibility, and creativity.
