Bilingual Chinese/English Case Manager, Seedco

Type: Part-time (24 hours per week)
Start Date: As soon as possible

Background of Program: High-profile workforce development and asset building pilot program. In this role, the Case Manager will be responsible for managing a caseload of Section 8 clients and for providing capacity building and mentorship to Case Managers. Case Manager will be based at a Community Partner Organization in Brooklyn.

Responsibilities:

  • Act as a primary case manager to assigned caseload. Case management activities include: carrying out initial client intake and needs assessment; screening clients for public benefit eligibility; developing individualized client service/career plans; and generating referrals for appropriate client training, job placement, financial education and family-based support services, e.g., child care, summer camp, food pantry, etc.
  • Responsible for following up with clients after a referral.
  • Create career plans with clients that focus on job placement and retention, as these are the primary goals of the program.
  • Track and report on client progress through completion of ongoing client progress notes, referral forms, and documentation of client benefits obtained and/or job retention or education goals met.
  • Conduct regular outreach activities to motivate and (re)engage clients.
  • Facilitate training and mentoring on case management methods and best practices for Case Managers through one-on-one and group sessions.

Qualifications:

  • Bachelors degree in social work or related field; MSW preferred.
  • At least 3-5 years relevant case management and/or direct service experience.
  • Excellent written/oral presentation and interpersonal skills.
  • Attention to detail and the ability to manage a high-volume caseload.
  • Bilingual in Cantonese, Mandarin, and/or English a plus.

Compensation: Hourly wage, commensurate with experience. No benefits. Seedco is an EEO employer.
