Closing the Divide: How Medical Homes Promote Equity in Health Care
According to a new report from the New York City-based Commonwealth Fund, many racial and ethnic disparities in health care could be eliminated by providing patients with access to a "medical home," which the report's authors define as a healthcare setting that offers well-organized care as well as after-hours advice and assistance. Based on a survey of more than 2,830 adults, the report, Closing the Divide: How Medical Homes Promote Equity in Health Care (56 pages, PDF), also demonstrates how having stable insurance and a regular provider improves health care access and quality among vulnerable populations.
