Living Wills In Choices: Living Well at the End of Life (44 pages, PDF), the Cuyahoga County Bar Association, in conjunction with the Ohio Hospice and Palliative Care Organization and the Ohio State Bar Foundation, has assembled an information kit about living wills — the means by which people can document the type of care they would like to receive if they become permanently unconscious or terminally ill and are unable to communicate — and healthcare power of attorney. The kit supplies facts about living wills, answers to frequently asked questions, details about filling out living will and power of attorney forms, and specifics for the state of Ohio, including downloadable advance directive forms.
