Mission:
The Institute for Urban Family Health is a non profit organization dedicated to the development of innovative ways to provide primary health care services to underserved urban populations based on the family practice model of care. Most Institute projects are developed and operated collaboratively with community-based organizations, hospitals, and medical schools. The goals of the Institute are to provide high quality, cost-effective primary care; sponsor innovative educational and training programs for health professionals; engage in primary care health services research; and participate in national, state and local health policy development.

... developing and operating family health centers
The Institute operates 13 health centers and nine healthcare sites for the homeless throughout the Bronx and Manhattan. Many of these centers were built in collaboration with community-based organizations, and all set a high standard for community health care. Our family physicians, internists, pediatricians, family nurse practitioners, social workers, and other specialists provide a full range of primary care services for adults and children, including reproductive health and prenatal care. The clinical staff manages access to specialty services, referral to alternate therapies, and care for patients when they are hospitalized. Dental care, social services, mental health counseling, care and counseling for individuals with HIV -related illnesses, and free clinics for the uninsured are also offered.

...training health professionals and other health care workers
The Institute offers several training programs to strengthen the ability of health professionals to provide high quality care to people in underserved communities, and to promote diversity in the healthcare workforce through programs geared to underrepresented groups.

  • The Faculty Development Program in Urban Primary Care, federally funded since 1983, trains future health care leaders from the tri-state area in topics such as health administration, educational theory, leadership, evaluation, diversity and cultural competency.
  • The Beth Israel Residency in Urban Family Practice is a 25-resident family practice program operated in collaboration with Beth Israel Medical Center, a world-class teaching hospital.
  • At the New York Metropolitan Regional Office of the New York State Area Health Education Center System, the Institute has expanded opportunities for community-based clinical training of future health professionals. The AHEC program recruits students from underrepresented and disadvantaged populations who will choose to work in underserved communities.

...innovation and leadership
Dr. Neil CaIman, President and CEO of the Institute since its inception, has received numerous national and state awards for his leadership in the development of primary care policy. Institute leaders serve on national, state and local government advisory boards.

The Institute is the first free-standing primary care facility in New York to implement an electronic medical record throughout its network of practices and the first to develop free clinics as outreach to the uninsured. It also established the first family practice residency program in Manhattan.

... making health equality a reality
The Institute is the lead organization in a coalition to eliminate racial disparities in health outcomes in the southwest Bronx, focused on the prevention and management of diabetes and cardiovascular disease. Bronx Health REACH, supported by grants from the Centers for Disease Control and Prevention and the New York State Department of Health, includes community-based organizations, hospitals and faith-based groups.

Providing Safety Net Services to Special Populations
The Institute is recognized for its leadership in the field of community health through the following services:

  • Social Services - including counseling, case management, insurance eligibility screening, and referrals for substance abuse treatment.
  • Care for the Homeless - providing health care, social services, outreach and health education to homeless people.
  • HIV Services - comprehensive services including medical care, mental health, treatment adherence, and health education to people affected by HIV.
  • Care for the Uninsured - the Institute collaborates with medical students from Albert Einstein College of Medicine and New York University School of Medicine to operate two free clinics that provide primary care and social services to the uninsured.

Funding Needs:
The Institute seeks support in the following areas:

  • programs to eliminate racial and ethnic disparities in health outcomes
  • expansion of our electronic medical record system
  • mental health care programs for low-income and uninsured patients
  • general operating expenses
