Narrative (not to exceed two pages):
Organization and/or project description, problem being addressed, proposed goals and strategies. Be sure to provide principal contact information including name, address, telephone, and e-mail.
Budget Information:
Annual operating budget if requesting general operations support; both project budget and organization budget if requesting project support. Explain sources of income, projected and actual. (No audited financial statement packets, please.)
Attachments:
Copy of IRS 501(c)(3) letter; list of Board of Directors; optional additional materials such as press clippings or brochures. (Please keep to a minimum, and do not send tapes or CDs.)
