Please print out the application form, fill it out, and mail it to the Foundation with the required attachments. The Foundation also accepts the Common Grant Application Format
developed by Grantmakers of Western Pennsylvania, if accompanied by the required attachments.
The application form is available in Adobe Acrobat/PDF format, which can be viewed and printed out using Adobe's Acrobat Reader, which is available from the Adobe web site free of charge.
