HOSPITAL MORALE GRANT PROGRAM
Application Guidelines
The Hospital Morale Grant Program is designed to build the morale of hospital and community-based medical personnel, with a special emphasis on the human and organizational factors affecting the morale of these care-giving employees. The program is grounded in the belief that improving the morale of those who work with patients will improve the quality of patient care.
Proposals seeking support for Hospital Morale projects in the Washington, DC area should include the following:
- A description of the organization's morale issue to be addressed, including the role of staff in identifying the issue; a statement of need for the proposed program including the population to be served.
- An explanation of the conceptual or theoretical framework underlying the program being proposed to address the stated issue.
- A description of measurable program objectives.
- A detailed description of the proposed program, including the involvement of staff in developing the program; specific strategies that will be used; activities that will be implemented.
- A line-item budget (with corresponding dollar amounts) specific to the amount being requested from the Hospital Morale Grant Program, accompanied by a budget narrative explaining how the funds requested will be spent.
- If different from # 5, a line-item budget (with corresponding dollar amounts) specific to the total budget for the proposed program, accompanied by a budget narrative.
- A description of the qualifications of principal staff for the proposed program (include copies of resumes); job descriptions of those to be hired.
- A plan for evaluating the proposed program (including measurement of the program objectives described earlier). Please include a description of the evaluation methods, including who will conduct the evaluation and qualifications of the evaluator(s).
- A plan for sustaining the program into the future.
- A work plan and a timetable describing how the project will be implemented.
- A list of other pending applications for funding to support the proposed program, especially other foundations to which the proposal has been submitted. If this is not a new program, please provide a list of previous funding support, in addition to the list of pending applications.
In addition to the proposal, every application should include:
- An original copy of the Grant Application Cover Sheet (with original signatures).
- An executive summary of the proposal.
- Two copies of the organization's 501 (c) (3) tax-exempt letter.
- A copy of the organization's latest annual report or audited financial statements.
- A copy of the organization's operating budget for the current and previous fiscal year.
- A list of the organization's Board of Directors
- A list of the organization's foundation and/or corporate support for the current and previous fiscal year (with corresponding dollar amounts for each).
- Background information on the applicant organization including the mission, programs and services offered, and recent accomplishments.
SUBMITTING THE PROPOSAL
Please submit one copy of the proposal, including attachments. Narratives should be no longer than 10 double-spaced pages, using 12 point fonts and 1" margins.
The deadline for proposal submissions in 2008 is Tuesday, September 2, 2008. All proposals must arrive at the Trusts' Washington office by 5 PM or be postmarked on that date.
The Prince Charitable Trusts will consider only one proposal a year across all program priorities from any organization.
