• Creation of new ways to address existing critical issues

• Desire to improve and enrich lives of individuals (rather than capital improvements)
• Original, unusual, dynamic and focused proposals

Proposals should include:

• Background information on organizational history, structure, general finances, board and staff
• Detailed description of proposed project with goals and objectives, target audience/participants, organizational structure, timelines, sites, impact and sustainability, and evaluation plans
• Financial information, which should include:
  • most recent audited financial report on organization;
  • budget vs. actual income and expenses for current year to date;
  • projected organization budget for next year;
  • program budget for each year of funding request with indication of total program cost and allocation of Kornfeld grant request;
  • copy of 501(c)(3) determination letter;
  • first page of most recent IRS Form 990.
