APPLICATION GUIDELINES

Letter of Inquiry (two pages maximum)

  • A brief description of the project for which the organization is seeking funding, including the project title and total project budget.
  • Name, title, address, and telephone number of the primary contact person.
  • Total amount of funding requested from the Lisa and Douglas Goldman Fund.
Please attach the following information:
  • A list of other sources of secured and potential funding for the project, including grant amounts and in-kind support.
  • A one-page itemized project budget.
  • A list of the organization’s officers, directors and/or trustees, and their primary affiliations.
Full Proposals

Applicants who are invited to submit a formal proposal should supply the following additional information:

  • A concise summary outlining the organization’s purpose, history, and recent accomplishments.
  • A detailed description of the project for which funding is requested, including the project’s objectives, relevance to the organization and community, evaluation criteria, time line, and budget.
  • Current financial statements (audited preferred); most recent annual budget, including revenues, expenses, and number of FTE staff; and current balance sheet.
  • Names and qualifications of project staff.
  • A list of recent and pending grants for the project, indicating foundation name and amount granted or requested.
  • Names, telephone numbers, and affiliations of three references who are familiar with the organization’s work and the project for which funding is requested.
  • Copies of federal and state tax-exemption letters.
  • Any other supporting materials, such as letters of support, news clippings, or brochures that would be helpful in evaluating the proposal Please do not send videotapes, CDs, or DVDs.
