Gifts to the Foundation

DONATIONS TO THE FOUNDATION THROUGH THE DOCTORS HOSPITAL "TREE OF LIFE"

Located in the main lobby of Affinity Medical Center in Massillon, Ohio, is a replica of a tree consisting of gold leaves. Each leaf is engraved with the name or names of those who have made a $500 or more contribution to the Austin-Bailey Health and Wellness Foundation, which was formed when the not-for-profit hospital was sold to a for-profit organization in late 1996. Donations less than $500 are gladly accepted, but only those contributions of $500 or more will be recognized with an engraved leaf on the Tree of Life.

All donations are used by the Foundation to promote the physical well-being of the citizens of Holmes, Stark, Tuscarawas, and Wayne Counties, and to enhance the education of people in the healthcare field.

Since the Austin-Bailey Health and Wellness Foundation is a non-profit 501(c)(3) organization and is tax-exempt, your contribution will be tax-deductible.

Checks in any amount should be made payable to the Austin-Bailey Health and Wellness Foundation, and mailed to 2719 Fulton Drive NW, Suite D, Canton, Ohio 44718.
